The State Institution "Kyzmat" informs about the possibility of online submission of an application for changing surname, first name, and patronymic.
To submit an application, it is necessary to provide the following information:
- the reason for changing the full name;
- the new surname, first name, and patronymic;
- contact phone number and email address;
- the chosen Public Service Center (PSC) for receiving the document;
- the period during which the application will be considered;
- preference for the delivery of the certificate (if desired).
The application will be reviewed within the specified time frame, and the applicant will receive a notification about the result. In case of approval, a code and information about the cost of the service will be sent.
The cost of changing the full name varies from 346 to 3172.51 soms.
