Autonomy of State Hospitals. Prices for Paid Services Will Be Formed Independently and May Increase by 35-50%

The Ministry of Health has introduced new mechanisms for state medical institutions that will allow them to operate with increased autonomy in management and finance. The draft resolution is currently being discussed by the public.

In November, President Sadyr Japarov approved a decree granting financial autonomy to certain state healthcare institutions. Health Minister Kanbek Dosmambetov noted that 20 state medical institutions will participate in the pilot project.

According to the new project, medical institutions will be able to pool their financial resources, redistribute them based on priorities, and independently establish their organizational structure and staff numbers based on actual data regarding operations, patient numbers, and services provided. Financial autonomy, including the ability to retain unspent funds at the end of the year, will allow for the creation of reserve funds for equipment renewal and modernization of institutions.

The Ministry of Health assures that the proposed tariff calculation methodology will not lead to unjustified increases in the prices of medical services. It will ensure fair pricing considering factors such as profitability, inflation, and rising utility costs. Currently, the cost of services in state institutions is 2-3 times lower than in private medical centers.

Preliminary estimates indicate that prices for paid services may increase by 35-50%, but will still remain approximately half of the prices for similar services in private clinics.

The tariff formation methodology, developed by the Ministry of Health in collaboration with the Ministry of Economy, will serve as the legal basis for state medical institutions to independently set prices within established norms and considering various costs.

Additionally, the project introduces a new wage payment order and a results-oriented incentive system while maintaining guaranteed salaries. Institution leaders will be able to set payment levels based on employee performance, which should contribute to increased motivation and professional growth of healthcare workers.

To attract additional financial resources, institutions may use donor and sponsorship funds by opening accounts at "Eldik Bank." They will also be able to independently set tariffs for paid services, taking into account mandatory benefits for socially vulnerable groups.

Main provisions of the project:

Organizational structure and staff: institutions have the right to form their structure and determine the number of employees based on actual needs, service volume, and patient numbers.

Financial autonomy: organizations will be able to manage revenues and freed resources, including funds from paid services, grants, and sponsorship assistance. Accounts may be opened at "Eldik Bank" for transactions. Unspent funds will be carried over to the next financial year.

Paid services: institutions will be able to expand the range of paid services, create separate divisions for their provision, and set tariffs according to the approved methodology. These tariffs must be transparent and published for patients, and include benefits for socially vulnerable categories.

Wage payment and incentives: a transparent system with guaranteed and performance-based incentive components is being implemented. Leaders are responsible for the accuracy of calculations and the timeliness of payments.

Budget and reporting: a consolidated budget is formed, institutions independently determine spending priorities, maintain accounting, and provide reports to the Ministry of Health and the Mandatory Health Insurance Fund.

Monitoring and evaluation: the Ministry of Health will evaluate the results of the pilot project on a quarterly basis regarding quality, accessibility, and efficiency of resource use.
